點滴是生命（「本會」）按照《個人資料（私隱）條例》（香港法例第486 章）收集及使用你的個人資料。

收集資料目的
本會可能會為下列其中一個或以上的目的，使用閣下所提供的個人資料：
- 處理及回應你的查詢、意見及建議；
- 處理及管理你於本會的報名及捐款；
- 處理及確認你參與本會活動及產品購買和服務的申請；
- 向你發出收據；
- 向你發出與捐款相關的資訊；
- （在你有權拒絕的前提下）告知你本會慈善項目相關的計劃、活動、籌募呼籲及其他項目；
- 進行研究及統計，以提升本會的服務及活動質素；及
- 法例容許的其他直接有關目的；
轉交及披露
本會只在獲得你同意的前提下，向本會在香港或境外的服務供應商轉交及披露你的個人資料，以使有關方面可代表本會向你傳送上文第1(f)段所述的推廣材料，並為本會提供後台、行政、通訊或資訊科技服務（包括但不限於處理信用卡的付款網關、進行捐款滿意度調查的調查公司及代表本會致電推廣及每月捐款的代理）。本會絕不會將你的個人資料提供予任何第三方，令該方向你推銷其產品或服務。

你須在註明「*」的欄目上提供所需的個人資料。如你未能提供，本會可能無法處理或管理你的報名、捐款或申請。
查閱及更正資料
你有權查閱及改正本會所持關於你的個人資料。

Personal Information Collection Statement

Purpose Your personal data is collected by ADOL for the purposes of:
- Processing and communicating with you regarding your enquiries, comments and suggestions;
- Processing and administrating any registrations and donations made by you to ADOL;
- Processing and confirming your application for our events, products and services;
- Issuing your donation receipt to you;
- Informing you regarding the usage of your donation;
- Informing you (subject to your applicable rights to decline) regarding charity campaigns, activities, fundraising appeals and other events organised or administered by ADOL;
- Conducting research and generating statistics to enhance the quality of ADOL’s services and activities; and
- Other directly related purposes as permitted by law;
Transfer and Disclosure
ADOL shall only transfer and disclose your personal data to service providers of ADOL (within or outside Hong Kong) to enable them to send to you (subject to your applicable right to decline) marketing materials described in paragraph 1(f) above solely on behalf of ADOL, and to provide back-end, administrative, communication, or IT services to ADOL (including but not limited to payment gateways for credit card processing, research companies for donor satisfaction surveys, and agents to make promotional and pledge donation calls on behalf of ADOL) and for other purposes related to ADOL’s functions and activities. ADOL does not provide your personal data to any third party for the purposes of enabling that third party to market its own products or services to you.

You must provide the information marked with an asterisk on this form (*). If you do not provide this information to us, then we may not be able to process or administrate your donations or applications.
Access and Correction of Data
You have the right to request access to or correction of your personal data held by ADOL.
